I understand that it is my responsibility to ensure that my child is adequately supervised by an adult at these times.

I understand that students are required to be respectful in language and behaviour throughout the session. If my child persistently disrupts the session, they will be removed from the call.

I will ensure that my child is located in a shared room (where possible, not a bedroom), be appropriately dressed (no pyjamas), and conduct themselves as they would in virtual sessions run by their school.

I understand that whilst it is advised that a responsible adult is present in the room during virtual lessons, they should not interact with other participants or disrupt the session in any way, and should ensure that no inappropriate language or disruptive noise can be heard in the background.

I understand that neither I or my child should record any of the virtual lesson or distribute virtual lesson content as consent has not been granted for this.

I understand that any safeguarding concerns will be reported to the designated member of staff at my child’s school.

I hereby consent to the Empathy Week’s recording of my child's name, image and voice during participation in distance learning through Zoom, as well as any personally identifiable information my child may share during instructional activities; and agree to the subsequent release of such recording(s) used as part of promotional materials for Empathy Week, online and in press media.

By ticking this box, I confirm that I am the parent or legal guardian of the student named below, and hereby fully release and discharge Empathy Week and its officers, employees, and agents from any and all debts or liabilities arising out of or in connection with the above-described uses of my child’s name, image or voice, or personally identifiable information shared by my child during instructional activities for the purposes of Empathy Week.
